What Is Adult Foster Care (AFC) program?

The Adult Foster Care (AFC) program, also known as Adult Family Care, is a program designed to help individuals who cannot safely live alone due to a medical, physical, or intellectual or developmental disability.  AFC provides support services and a stipend to families for the care provided to their loved ones.

Is My Loved One Eligible for Adult Foster Care

Individuals who are unable to safely live alon, are at least 18 years, Medicaid, Medicare or Private insurance pay, and have mental illness and developmental disabilities requiring daily cueing and supervision or physical assistance with at leat one activitiy of daily living, may be eligible to receive AFC services.

What Is An Activity of Daily Living (ADL)?

Activities of daily living are basic self-care tasks.  These tasks include bathing, dressing, toileting, eating, walking, or transferring from one posiition to another.

Are caregivers qualified?

Eligible caregivers must be a caregiver aide or Certified Nurses Assistant (CNA).  They must be able to devote appropriate time to provide care and ensure the individuals safety.  Caregivers are required to be 18 years of age or older, pass a criminal history background check, and be free of mental, physical or other impairments that would interfere with caregiving.  Caregivers are non-related, cannot be the resident's legal guardian or spouse.

How Will My Adult Foster Care Agency Support Me?

AFC agencies use a person-centered approach and provide ongoing support from a care manager and registered nurse to ensure the individual's care needs are being met, including advocacy, care management, and clinical support.  AFC. staff members provide specialized trainings, which include medication managementand emergency procedures.

The next step is to see if you, your loved one are eligible to receive services.  The Adult Services Worker will give you an application to complete.  After completion, the adult services worker will assist you with services and housing placement.

Are There House Rules at an AFCHome/

Yes.  The administrator will give you house rules when you are admitted into our home.  House Rules are expected to be followed by residents and caregivers.  If rules are not followed by the resident, he/she will be discharged from our home.
